Please download the tuition Application Form or obtain from CDAC One-Stop Service at 65 Tanjong Katong Road Singapore 436957 during office hours.

Please make a photocopy of ALL of the following documents:

• First page of POSB / DBS passbook that bears the personal information of the account holder,
• Student's year-end examination results slip,
• Birth Certificate (BC) / NRIC of student applying for tuition and
• NRIC / BC of all family members who are staying at the same address.
  

When the above are ready, proceed to:

• Fill in the application form carefully and
• Bring along the completed application form and the above photocopied documents to apply at your choice of tuition centre.
  

Please note:

• Application form that is incomplete or without photocopied documents will NOT be processed.
• All late applications are to be sent by post to CDAC Secretariat.
• Admission is subject to availability of tuition places.
